People living in the TQ12 postcode area can register at Devon Square Surgery. You will need to fill in a registration form, which you can do online, or in person at the surgery.
We have a practice booklet which is available from the front desk of the surgery, which contains useful information about our services.
Medical treatment is available from the date of registration, although we are unable to issue medication until you have seen a practitioner. Please contact reception for further information.
At Devon Square Surgery the doctors operate a personal list, meaning each patient has a registered 'usual' doctor. Your registered doctor can be found on any prescription issued to you and on any correspondence we send you. If you are unsure who your registered doctor is please ask at reception.

Click on the links above to open the form. When you have completed all of the details on the form, click on the "Send" button to send your form to us.
When you visit the surgery for the first time you will be asked to sign the registration form and confirm that the details are correct.
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you can print off a Registration Application Form and New Patient Health Questionnaire (below), fill them out and return them to us as soon as possible.
1/ Registration Form
There is a telephone based translation service that the doctors and nurses can use during clinical consultations to aid communication.
Please let us know that you require this service when making the appointment, and which language you require, so that the doctor can pre arrange the telephone translation service.
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
